Please submit this registration form to enter your class/school into the poster competition. The competition is limited to the first 50 schools that submit a complete registration form. Once we have received your registration form we will send you a pack containing further details about the competition, plus certificates and Smoke Free Manchester wrist bands for every child that takes part in the competition.
